Requirements:
1. Application letter
2. Fully accomplished Personal Data Sheet CS Form 212 (Revised 2017)
3. Curriculum Vitae
4. Photocopy of Transcript of Record (if applicable) or Form 137
5. Photocopy of Diploma/ or Certificate of Graduation (if applicable)
6. Photocopy of an authenticated copy of the certificate of eligibility/ bar or board rating/license (if applicable)
7. Photocopy of certificate of employment (if applicable)
8. Photocopy of certificate of training/seminars attended (if applicable)
9. Photocopy of Performance rating in the last rating period(if applicable)
