1. Fully accomplished Personal Data Sheet (PDS) with a recent passport sized picture

(CS Form No. 212, Revised 2025), with the required Work Experience Sheet, which can be downloaded at www.csc.gov.ph

‼️USE THE 2025 PERSONAL DATA SHEET when applying for any positions available.

2. Performance rating in the last rating period (applicable for applicants for transfer and promotion)

3. Certificate of Eligibility (for CS Eligibles); PRC License for PRC Board Passers; Board Rating for applicants to positions that do not require practice of profession.

4. Diploma for Primary, Secondary and Grade12/Senior Highschool Graduate for positions applicable and Transcript of Records.
