Due to the combined effects of Tropical Depression “Mirasol” and the approaching Super Typhoon “Nando,” all classes at all levels—public and private, including night classes—as well as work in both government offices and private institutions across the Province of Sultan Kudarat are hereby suspended.

“Mirasol” has already brought heavy rains, flooding, and landslides in some areas, while “Nando” is expected to bring even stronger winds, torrential rains, and possible storm surges in the coming hours.

However, the suspension does not cover government offices and private entities involved in essential services such as healthcare, disaster preparedness and response, road clearing, rehabilitation, and other vital operations necessary for public safety.

Everyone is advised to stay safe, remain indoors, and keep updated with official advisories.

PDCA Problem-Solving Implementation Guide
1. Record the Problem
Clearly define the issue by addressing the following:
✅ What? – Define the problem. Example: "Machine downtime due to overheating."
✅ Where? – Specify the location. Example: "Production Line 3."
✅ When? – Mention the time or frequency. Example: "Every 3 hours during peak operation."
✅ Who? – Identify the team/person involved. Example: "Maintenance team and machine operators."

2. Analyze the Problem (Fishbone/Ishikawa Diagram)
Identify potential root causes under six categories:
1️⃣ Man (People) – Human-related issues like skill gaps. Example: "Lack of training on temperature monitoring."
2️⃣ Machine (Equipment) – Equipment failures. Example: "Cooling fan failure due to wear."
3️⃣ Management (Policies & Supervision) – Leadership and procedures. Example: "No preventive maintenance schedule."
4️⃣ Method (Process & Procedures) – Inefficient processes. Example: "Ineffective lubrication process."
5️⃣ Material (Resources) – Poor quality materials. Example: "Low-quality lubricants."
6️⃣ Milieu (Environment) – External factors. Example: "High temperatures affecting machine performance."

3. Identify Root Causes (5 Whys Technique)
Use the 5 Whys technique to drill down to the root cause:
❓ Why is the machine overheating? → "Cooling fan failure."
❓ Why did the fan fail? → "Not replaced on time."
❓ Why was it not replaced? → "No preventive maintenance plan."
❓ Why is there no plan? → "Management did not prioritize it."
❓ Why did management not prioritize? → "Lack of awareness about maintenance importance."

4. Take Action (Corrective & Preventive Measures)
Define the actions and responsibilities:
✅ What? – Example: "Implement preventive maintenance schedule."
✅ Who? – Example: "Maintenance Supervisor, John Doe."
✅ When? – Example: "By 30th September 2025."

5. Validate the Results
Evaluate the outcome:
✅ Result Evaluation:

Good, on target – Problem resolved.

Slightly improved – Needs further work.

Bad, off target – Issue persists.
✅ Standardization: Update procedures if needed.
✅ Approval: Score effectiveness and get expert approval.
