⚠️FOR PROXY BENEFICIARY
For identified beneficiaries who are deceased, incapacitated, bedridden, suffering from serious illness, or otherwise unable to personally claim the assistance, the next qualified household member appearing in the Household Roster may be authorized to receive the assistance on behalf of the household, subject to validation and presentation of the following documents:
1. Authorization Letter Signed by the Primary Beneficiary indicating the following: (Name of the authorized representative, Relationship to the beneficiary; and reason for the beneficiary's absence during payout)
2. Original (handwritten and with original signature)1 Photocopy
3. Valid Government-issued ID (with 3 specimen signatures); *Primary Beneficiary: 2 photocopies*Proxy: 2 photocopies
4. Proof of Relationship; *Marriage Contract - (if the proxy is a spouse of primary beneficiary)*Birth Certificate - (if the proxy is the child of primary beneficiary)*Certificate of Cohabitation & Birth Certificate (one child)- (if the proxy is a common-law partner bene)
5. Death Certificate -(if the primary beneficiary is deceased)
* In case the death certificate is not yet available, a certification issued by the barangay confirming the death of the beneficiary.
NOTE: ‼️Minor proxies are not allowed.Only representatives listed in the Household (HH) Roster may claim on behalf of the Primary Beneficiary.
⚠️FOR BENEFICIARIES WITH NAME DISCREPANCIES
-If the name on your Valid ID is slightly different from the name on the Official Beneficiary List (e.g., JUAN SANTOS DELA CRUZ vs. PUAN SANTOS DELA CRUZ), secure a Certificate of Oneness from your Barangay before the payout schedule.
⚠️ FOR BENEFICIARIES WITH LOST EBT CARD
-Submit 2 Photocopies of Panunumpa
⚠️ FOR MINOR BENEFICIARIES WITH NO VALID ID
-School ID (Validated) - 2 photocopies
-Birth Certificate - 2 photocopies
