NOTICE OF ACCREDITATION TERMINATION/NON-RENEWAL
Please be informed that the following healthcare facilities in Region 4A have been formally revoked or have not renewed their accreditation under the PhilHealth YAKAP Program:
1. Tagaytay Medical Center, Inc.
2. Alfonso Municipal Health Office
3. Adventist University of the Philippines
4. Tanza Specialist Medical Center
5. Municipal Health Office – Ternate
6. Medeek Ambulatory Surgical Clinic
7. South Imus Specialist Hospital, Inc.
8. Ospital ng Cabuyao
9. Thaddeus Maternity Clinic
As such, these facilities are no longer authorized to provide PhilHealth YAKAP Package services, including:
• Free primary care consultations
• Medical check-ups and preventive health services
• Outpatient medications covered under the program
Guidelines for Affected Members
To ensure continued access to primary healthcare benefits, affected members are advised to observe the following:
1. Immediate Change of YAKAP Provider
Members currently registered with any of the above-mentioned facilities may immediately transfer to another accredited YAKAP Clinic. The usual restriction on provider transfer during the fourth quarter of the year shall not apply in this case. Members are encouraged to select an accredited YAKAP provider nearest their residence to avoid disruption in accessing healthcare services.
2. For assistance and inquiries, please contact the nearest PhilHealth Office or visit the official PhilHealth channels.
We encourage all affected members to update their YAKAP provider at the earliest opportunity to ensure uninterrupted access to their healthcare benefits.
