๐ REQUIREMENTS FOR NATIONAL ID REGISTRATION:
๐น 0 TO 4 YEARS OLD:
โข Original Birth Certificate (Local/NSO/PSA issued) OR Barangay Certification containing:
- Full Name (First, Middle *please spell out your complete middle name, Last Name)
- Birthday
- Birth Place (Municipality and Province)
- Complete Address (with Sitio/Zone/Street)
- Parentsโ Full Name
โข Presence of either Parent or Guardian, plus their respective National ID
(Note: Biometric data will be captured from the accompanying parent/guardian for children aged 1-4 years old)
โข For Digital National ID: Download the EgovPH App
๐น 5 YEARS OLD AND ABOVE:
โข Original Birth Certificate / Any Valid ID OR Brgy. Certification Containing:
- Full Name (First, Middle *please spell out your complete middle name, Last Name)
- Birthday
- Birth Place (Municipality and Province)
- Complete Address (with Sitio/Zone/Street)
