• Form 138-A or Transcript of Records(photocopy)
• Birth Certificate (photocopy)
• SHS or College Diploma (photocopy)
• NBI, Police or Barangay Clearance (any of the following available documents)
• Marriage Certificate (for married women)
• 1x1 picture(2) and 2x2 picure(1)
