Reminder to all local treasurers, budget officers, and assessors on the extension of the deadline for the submission of Q1 FY2023 reports and the timely submission of Q4 FY2022 reports in the LIFT System.

Concerned officials are encouraged to continue strict observation of the deadlines for their respective reports.

SOCIAL PENSION PAY-OUT REQUIREMENTS: (please prepare 2 copies of every requirements)

FOR PERSONAL CLAIMNANT

1. Present the original and photocopy of the OSCA ID

FOR AUTHORIZED REPRESENTATIVE

1. Present the original and photocopy of claimant’s ID

2. Present the original and photocopy of beneficiary’s OSCA ID

3. Authorization letter from the beneficiary indicating the name and relationship of the authorized representative and the reason/s of absence, signed by the beneficiary and claimant. Attested by the C/MSWDO & OSCA. (Please use the prescribed format of authorization letter)

4. Picture of the beneficiary holding the latest calendar (OCTOBER - DECEMBER 2022)

5. Proof of relationship - Dokumento nga nagpamatood sa koneksyon sa relasyon sa pensioner ug ang gi authorize (Birth certifcate, Marraige contract, if not available Barangay Certificate of Identity sign by Brgy Captain attested by OSCA HEAD & MSWDO.

FOR DECEASED SOCIAL PENSIONER
(OCTOBER - DECEMBER 2022 - 4th quarter)

1. photocopy of the death certificate of the beneficiary

2. photocopy of the beneficiary and claimant’s ID

3. Warranty and release from liability (Attested by the C/MSWDO and OSCA)

Please Note:
>Lockdown reason for Authorization will no longer be allowed kay wala nay lockdown karun, and at d same time dili napud pwde ang Authorization letter nga through messenger,.. Dapat original ang tanang Authorization letter.

>Palihug usab isulat sa isa ka photocopy ID ang Edad, unsay gamitan sa kwarta nga dawaton, Tribo, ug kun PWD ba o Dili.

>Magsul-ub ug facemask para sa kaugalingog safety
