In compliance with Memorandum Order MPL No. 5-2019 issued by the Provincial Assessor of Misamis Oriental dated October 3, 2019. Any person requesting for the issuance of CTC of Tax Dec, Property Historical Ownership, Tax Map and other assessment records, must present the following requirements:
1. Valid I.D., if the requesting party is the declared owner and co-owner of the Property.
2. Duly Notarized SPA/Authorization Letter, if the requesting party is not the owner of the property accompanied by Valid I.D. of the property owner and his/her authorized representative.
3. Deed of Conveyances and Valid I.D. for property subject to transfer of Ownership.
4. Any Proof of Possession and Valid I.D for property not covered by above described requirements.
