NATIONAL ID REGISTRATION and UPDATING/CORRECTION OF DEMOGRAPHIC INFORMATION โโโ

Requirements for REGISTRATION;
0-4 years old:
โ Original PSA/Local copy of Birth Certificate
โ Parent/Guardian's National ID
โ Authorization Letter (if parent is unable to accompany the child)
5-17 years old:
โ Original PSA/Local copy of Birth Certificate
18 years old and above:
โ 1 Gov't. Issued ID (Passport, Driver's License, Voter's ID/Certificate etc.)
Requirements for DEMOGRAPHIC UPDATING/CORRECTION
Original PSA/ Local Copy of birth certificate - if First, Middle and Last Name, Place of Birth, Date of Birth and Gender.
Original PSA/ Local Copy of marriage certificate - Marital Status
Certificate of Residency/ Barangay Certificate/ 1 Valid ID stating the address- Permanent Address
Blood typing result - Blood type
Other services offered:
- Issuance of ePhilId; bring your Transaction Slip
- Digital National ID assistance
- Correction of Information (yung mga may mali sa National ID) and Updating of Marital Status ( ex. Single to Married)

Note : 1-2 weeks after registration or updating ay maaari po kayong magtungo sa 1BOSSCO 2nd floor, The Bunker, Balanga City para kayo ay maissuehan ng EphilID (Temporary National ID). Samantala, ang PVC typed ID ay idedeliver nalang po sa inyong address sapagkat hindi po namin maaassure kung kelan ito dadating.
