1. Duly Accomplished Application Form with 1x1 picture.

This webinar will discuss the inclusion of the SK in the National Health Insurance Program (NHIP) of the Philippine Health Insurance Corporation (PhilHealth) and the other benefits and privileges covered under the NHIP, including the e-KONSULTA program. Further, this webinar will guide SK officials in how to obtain a PhilHealth Identification Number (PIN).
