O2 inhalation given via nasal cannula at 2 Lpm. CBG checked, revealed 188 mg/dL. SAMPLE History obtained. Advised for further care and management. Transported to hospital of choice. Upon arrival at the scene 2 patients were seen conscious and sitting on the side of the road.

Patient 1. Patient is Female, 24-year-old with the chief complaint of Multiple abrasion arms, nose bridge, forehead, upper lip, right shoulder, left knee and both feet, Punctured wound @ left knee, and swelling @ left forehead secondary to vehicular accident. (-) vomiting, (-) LOC. Physical assessment done. Wound dressing done. Vital signs checked and recorded. Advised for further care and management. Transported to Hospital of Choice. Endorsed to Nurse on Duty.

2. Patient is female, 23-year-old with the chief complaint of abrasion @ both knee and left hip secondary to VA. (-) LOC, (-) vomiting. Physical assessment. Vital signs checked and recorded. Wound dressing done. Advised for further care and management. Refused for transport. waiver well explained. Understood as claimed. Signed and Secured. Patient was then released.
