Patient is Female, 85-year-old with the chief complaints of DOB, Swelling both upper and lower Extremities. . Initial Assessment done. Vital signs checked and monitored. CBG taken and revealed 131 mg/dL. O2 inhalation given via nasal cannula at 6Lpm. SAMPLE History obtained. Advised for further care and management. Transported to Hospital of Choice. Endorsed to Nurse on Duty.
