Patient 2. Upon arrival at scene, Patient seen sitting. Patient is Male, 21 year-old, with the chief complaints of abrasion @ frontal area, avulsion @ chin, multiple abrasions @ arms and legs and laceration
@ right foot secondary to V/A. Physical assessment Done. Vital signs checked and recorded.Bp-120/90,RR-19 PR-100,Spo2-97%.Wound dressing done. Advised for further care and management. Refused for transport. Waiver well explained. Understood as claimed. Signed and secured. Released.
