Applicants must submit one copy in white folder of the ff:
1. Letter of intent addressed to the. Schools Division Superintendent
2. Duly accomplished PDS (CS Form No. 12, Revised 2017)
3. Photocopy of voter's ID or any proof of residency
4. Photocopy of Cert. Of Eligibility, Rating, and PRC license
5. Photocopy of Cert. of board rating
6. Photocopy of Transcript of Records
7. Photocopy of duly signed service record (if applicable)
8. Photocopy of valid TESDA NCIi, NCIII if applicable
9. Checklist of requirements and Omnibus Sworn Statement on Certification on the Authenticity and Veracity of the submitted documents
All photocopies must be certified photocopy of the original by the School Head upon review
