2. Performance rating the present position for one (1) year
(If applicable);
3. Photo copy of certificate of eligibility/rating/license; and
4. Photo copy of Transcript of Records

NOTE: APPLICANTS WITH INCOMPLETE DOCUMENTS SHALL NOT BE ENTERTAINED.
