Beneficiaries: Unpaid 1st and 2nd batch beneficiaries

Venue: Municipal Ground, Barangay 3, SFADS

Date: August 28, 2026

Time: 8:00 A.M onwards

What to bring:
1. Filled UPLIFT Forms (no erasures)
2. Photocopy of Valid Govt I.D preferably National I.D (w/ 3 Signatures)
3. If proxy, attaching documents necessary

Note:
1. If ang beneficiary nga nalista is WGP ug 4Ps na, dili na allowed makakuha. So, dili na sila paadtuon.

2. Allowed ang PROXY KUNG nga trabaho OUTSIDE Agusan Sur, PROVIDED naa sila sa Family Roster.

3. Bring own ballpen, water ug food.
