VISION SCREENING ASSESSMENT CONDUCTED AMONG LEARNERS OF VICENTA S. DAVID NATIONAL HIGH SCHOOL

On August 4, 2026, Vicenta S. David National High School successfully conducted a Vision Screening Assessment among consented learners from Grades 7 to 10 as part of the school's continuing commitment to promoting the health and well-being of its learners under the Oplan Kalusugan sa DepEd (OK sa DepEd) Program. The activity was facilitated by Kris Pauline M. Caballes, Nurse II, from the Schools Division Office of Laguna, with the primary objective of assessing the visual health of learners, identifying those who may require further eye evaluation, and ensuring timely intervention for learners experiencing possible vision-related concerns that may affect their academic performance and overall development. To minimize disruption to instructional time, the vision screening for Grades 8 and 9 was conducted before class hours, while the assessment for Grades 7 and 10 was carried out after class hours.

The screening covered a total of 95 learners from the junior high school department. Specifically, 21 Grade 7 learners, 23 Grade 8 learners, 34 Grade 9 learners, and 17 Grade 10 learners voluntarily participated in the assessment after submitting the necessary parental consent. During the activity, learners underwent standard vision screening procedures to determine their visual acuity. Those who exhibited signs of visual impairment or failed the initial screening were identified for appropriate referral to eye care professionals for further evaluation and management.

The successful implementation of the activity was made possible through the full support and leadership of Mrs. Juliet M. Bulahan, School Head, together with the cooperation of teachers, school personnel, parents, and participating learners. The Vision Screening Assessment reinforces the school's commitment to safeguarding the health of every learner by promoting early detection of vision problems, improving access to appropriate healthcare services, and fostering an educational environment where learners are physically equipped to participate effectively in classroom learning.

SCHOOL MAPPING AND HAZARD
MONITORING

In line with the school’s commitment to ensuring a safe and secure learning environment and in support of the Department of Education’s disaster risk reduction and management initiatives, the Student-Led Watch Team of Vicenta S. David National High School conducted a school hazard mapping activity on July 27, 2026, from 12:00 noon to 12:30 PM after class of morning session. The activity aimed to identify, document, and mark potential hazards within the school premises to raise awareness among students, faculty, and staff, and serve as reference for safety planning and emergency preparedness.

The activity was spearheaded by the Student-Led Watch Team, composed of student representatives from different grade levels, supervised by the School Disaster Risk Reduction and Management (SDRRM) Coordinator and designated faculty advisers.

Based on the School Hazard Mapping and Monitoring conducted on July 27, 2026 at Vicenta S. David National High School, a total of 10 hazards were identified within the school premises. Of these, 1 (10%) was classified as High Risk, 3 (30%) as Medium Risk, and 6 (60%) as Low Risk. The most critical hazard observed was the presence of broken windows in Kalaw Building Rooms 1-4, which pose an immediate safety threat to learners and personnel. Medium-risk hazards were primarily related to electrical safety concerns, including exposed electrical wires and defective electrical facilities, as well as broken door k***s that may affect safe access and emergency evacuation. Most identified hazards have already been reported; however, several remain unresolved and require immediate corrective action.

Priority Recommendations:
1. Immediately repair or replace all broken windows in Kalaw Building Rooms 1-4 and restrict access to affected areas until repairs are completed to prevent injuries.
2. Address all electrical hazards by covering exposed electrical wires, replacing defective plugs and light fixtures, and conducting a comprehensive inspection of the school's electrical system.
3. Implement and expedite maintenance and safety improvement measures for reported hazards, including replacement of broken door k***s, repair of damaged sinks and ceilings, installation of barriers for open pits, and strengthened monitoring of stray animals within the school grounds.

FAMILY EARTHQUAKE PREPARENESS
PLAN

The Philippines is situated along the Pacific Ring of Fire, making it highly prone to seismic activities. In line with the Department of Education’s thrust to integrate disaster risk reduction and management (DRRM) across the curriculum, Last July 27, 2026, the Grade 7 classes of Custodio and Bonifacio conducted an activity focused on drafting a Family Earthquake Preparedness Plan. This initiative was embedded in their Physical Education and Health class to connect personal safety, physical well-being, and family security. The activity aimed to equip learners with practical skills to protect themselves and their households before, during, and after an earthquake.

The activity was designed to enable the students to:
1. Identify potential earthquake hazards and risks at home and in the community.
2. Formulate a comprehensive family emergency plan covering communication, evacuation, and safety protocols.
3. Prepare a basic emergency “Go Bag” with essential supplies.
4. Value the importance of teamwork, readiness, and health consciousness in times of disasters.

The students were grouped by class and guided through the step-by-step process of creating their own family preparedness plan. Key components included:
-Family Communication Plan: Listing contact numbers of family members, meeting places (inside and outside the house), and designated persons to pick up children.
-Home Safety Check: Identifying heavy furniture, appliances, and hanging objects that may fall and cause injury; suggesting ways to secure them.
-Evacuation Procedures: Mapping safe exit routes from each room and the house, and identifying the nearest safe open space.
-Emergency Supplies: Listing items for a “Go Bag” such as water, non-perishable food, first aid kit, flashlight, whistle, and important documents.

Students actively shared ideas based on their home setups. Some volunteered to demonstrate proper body positioning during a quake, while others explained how to check for injuries applying health concepts learned in class.
