⏰ Time: 8:00 AM – 3:00 PM

Please bring the following requirements:

📌 OPEN HIGH SCHOOL PROGRAM
• Report Card
• Photocopy of Birth Certificate
• Any of the following (whichever is applicable):
• Employment Certificate (if working)
• Medical record/s (if pregnant or with illness)
• Photocopy of Child’s Birth Certificate (if with child)

📌 EVENING CLASS

• Photocopy of Birth Certificate
• Employment Certificate
