REQUIREMENTS:

🪪 PRIMARY DOCUMENTS:
• PSA-issued Certificate of Live Birth and one (1) government-issued identification document which bears a full name, front-facing photograph, and signature or thumb mark

• Philippine Passport or ePassport issued by the Department of Foreign Affairs (DFA)

• Unified Multi-purpose ldentification (UMID) Card issued by the Government Service lnsurance System (GSIS) or Social Security Systen (SSS); or

• Student’s License Permit or Non- Professional/Professional Driver’s License issued by the LTO.

(In case there is/are discrepancy/ies in the entry/ies in PSA-issued Certificate of Live Birth and the government-issued ID presented, the entry/ies in the PSA-issued Certificate of Live Birth shall prevail.)

aa. Retiree’s lD issued by the PNP, AFP and Philippine Coast Guard (PCG);

bb. Conductor’s License issued by the LTO;

cc. Philippine Veterans Affairs Office (PVAO) Pensioner’s lD (Veteran or Dependent);

dd. Seafarer’s ldentity Document (Seaman’s lD);

ee. Tribal Certificate/lD issued by the Tribal Affairs Office under the Office of the Mayor (if applicable);

ff. Certificate of Confirmation issued by the National Commission on lndigenous Peoples (NCIP) or Certificate of lndigenous Cultural Communities (lCCs)/lndigenous Peoples (lPs) Membership (CIPM) issued by the Tribal Leader of ICC/IP; or

gg. Certificate of Tribal\membership issued by the National Commission on Muslim Filipinos (NCMF).

Ang mga sumusunod na dokumento ay maaaring tanggapin bilang secondary document kung may litrato ng aplikante sa harapan, may pirma o thumbmark, buong pangalan, permanenteng address at araw ng kapanganakan ng aplikante.

Ang National ID program ay programa ng Phil. Statistics Authority (PSA) at PhilSys, sa pakikipagtulungan ng City Civil Registry ng Taguig City.

🪪 NAKAPAG-APPLY NA AKO NOON PERO WALA PA RIN AKONG NATATANGGAP NA NATIONAL ID. PUWEDE BA AKONG MAG-APPLY ULIT?

One time lang po ang registration sa National ID. Hindi na puwedeng umulit.

• Pumunta nang maaga. Only 300 slots are available for birth registration.
• Kung masama ang lagay ng panahon, maaaring ma-postpone ang MBR para sa kaligtasan nating lahat.

👉 REQUIREMENTS FOR ON-TIME BIRTH REGISTRATION
( Para sa mga sanggol na ipinanganak sa loob ng 30 araw pero wala pang birth certificate )

A. Kung KASAL ang magulang:

1. Certificate of Live Birth form (Makukuha ito sa City Civil Registry)
2. Marriage Contract
3. National ID ng magulang
4. Government-issued ID ng magulang

B. Kung HINDI KASAL ang magulang:

1. Certificate of Live Birth form (Makukuha ito sa City Civil Registry)
2. Affidavit of Admission of Paternity (Makukuha ito sa City Civil Registry)
3. Affidavit to use the Surname of the Father (Makukuha ito sa City Civil Registry)
4. National ID ng magulang
5. Valid ID ng magulang

👉REQUIREMENTS FOR LATE BIRTH REGISTRATION
( Para sa mga ipinanganak ng higit sa 30 araw ang nakalipas pero wala pang birth certificate )

STEP 1: Pumunta sa mga lokasyon ng Mobile Birth Registration o sa opisina ng City Civil Registry sa Taguig City Hall o at kumuha ng Certificate of Live Birth (COLB) form.

STEP 2: Sagutan (typewritten or computerized) ang mga detalyeng hinihingi sa Certificate of Live Birth Form at pirmahan. Sa likuran nito, sagutan
at pirmahan ang Affidavit for Delayed Registration of Birth. Ipa-notaryo ang COLB.

STEP 3: Kumuha ng negative certification mula sa PSA na nagpapatunay na wala pang birth certificate ang aplikante.

STEP 4: Kung KASAL ang magulang kumuha ng Certified True Copy ng Marriage Certificate mula sa Philippine Statistics Authority (PSA) or Local Civil Registry Office.

Kung HINDI KASAL, sagutan ang Affidavit of Acknowledgement/ Admission of Paternity. (Ito ay matatagpuan sa likuran ng Certificate of Live Birth Form) at ang Affidavit to Use the Surname of the Father (AUSF). Pirmahan at ipa-notaryo.

👉MGA KARAGDAGANG REQUIREMENTS:

1. National ID ng magulang
2. Valid ID ng magulang
3. Dalawa (2) sa mga sumusunod na dokumento bilang patunay sa pangalan ng mga magulang, petsa at kung saan isinilang ang aplikante:
a. Original Baptismal Certificate mula sa parokya
b. Medical Certification or Immunization Card mula sa ospital, health center o doktor
c. Certified True Copy ng Form 137/ School Records (para sa edad 7-17 taon)
d. Certified True Copy ng Voter’s Registration (para sa edad 18 pataas)

👉SINO ANG PUWEDENG MAG-FILE NG BIRTH REGISTRATION?

• Kung KASAL ang magulang, puwede kahit isa sa mga magulang. Puwede rin ang aplikante mismo kung siya ay nasa 18 taong gulang pataas.
• Kung HINDI KASAL, required ang parehong magulang na magpakita sa pag-file.
• Required din na sumama ang anak na ipaparehistro kung ito ay nasa 7- 17 taong gulang.

👉PAANO KUNG WALA ANG TATAY SA BIRTH REGISTRATION?

• Kung KASAL, hindi ito problema. Maaring mag-file ang nanay.
• Kung HINDI KASAL, hindi maaaring wala ang tatay, maliban kung siya ay namatay na, nasa ibang bansa o naglayas at inabondana ang pamilya.
• Sa ganitong pagkakataon, maaaring mag-rehistro ang nanay. Gagamitin sa birth certificate ang apelyido ng nanay.

👉PAANO KUNG WALA ANG NANAY SA BIRTH REGISTRATION?

• Kung KASAL, hindi ito problema. Maaring mag-file ang tatay.
• Kung HINDI KASAL, hindi maaaring wala ang nanay, maliban kung siya ay namatay na, nasa ibang bansa o naglayas at inabondana ang pamilya.
• Sa ganitong pagkakataon, maaaring mag-rehistro ang tatay.
• Puwede rin ang lola o nanay ng nanay ng ipaparehistro. Kailangan lang ng Affidavit of Guardianship.

👉PUWEDE BA SA LATE BIRTH REGISTRATION KAHIT HINDI ISINILANG SA TAGUIG?

Puwede po. Kung hindi isinilang sa Taguig ang ipaparehistro, tutulong ang opisina ng City Civil Registry ng Taguig sa pakikipag-ugnayan sa lokal na pamahalaan kung saan isinilang ang magpaparehistro. Ito ang tinatawag na Out-of-Town Registration.

👉MAGKANO ANG BAYAD SA LATE BIRTH REGISTRATION?

• Walang bayad ang mismong pagpaparehistro dahil ito ay libreng serbisyo ng Lungsod Taguig sa mga Taguigeño.
• Pero may babayaran sa pagkuha ng mga requirements tulad ng notarization, barangay clearance, at negative certification mula sa Phil. Statistics Authority (PSA).

ALTERNATIVE WORK ARRANGEMENTS FOR GOVERNMENT OFFICES, ALTERNATIVE LEARNING MODES FOR SCHOOLS ON AUGUST 17

Pursuant to Memorandum Circular No. 127, government offices are directed to shift to appropriate alternative work arrangements in the National Capital Region and the following provinces starting 12:00 noon on August 17, 2026:

📍 National Capital Region (NCR)
📍 Zambales
📍 Bataan
📍 Bulacan
📍 Pampanga
📍 Tarlac
📍 Nueva Ecija
📍 Cavite
📍 Batangas
📍 Rizal
📍 Laguna
📍 Quezon
📍 Occidental Mindoro

Unless otherwise directed by their respective Local Chief Executives, alternative learning modes for classes at all levels in public and private schools shall be implemented in the aforementioned areas.

Agencies involved in the delivery of basic and health services, disaster preparedness and response, and other vital services shall continue their operations and render the necessary services.

The suspension of work or adoption of alternative work arrangements in the private sector shall be left to the discretion of management.

PAANO KUMUHA NG DEATH CERTIFICATE SA TAGUIG CITY?

1. Ano ang Death Certificate?
Ang Death Certificate ay isang dokumento na nagsasaad ng oras, lugar, at sanhi ng pagkamatay ng isang tao.

2. Sinu-sino ang dapat kumuha ng Death Certificate?
a. Asawa
b. Anak
c. Magulang
d. Kamag-anak

Magdala ng valid ID o ibang dokumento na nagpapatunay na ikaw ay kamag-anak ng namayapa.

3. Saan kukuha ng requirements sa Death Certificate?

Maaaring kumuha ng forms at iba pang requirements sa opisina ng City Civil Registrar sa Ground Floor Taguig City Hall Annex.

Bukas ito MONDAY TO FRIDAY ( 7AM to 4PM ); SATURDAY AND SUNDAY ( 8 AM to 4 PM )

4. Ano ang proseso sa pagkuha ng Death Certificate?

A. KUNG NAMATAY SA BAHAY:

a. Kumuha ng apat na orihinal na kopya ng Municipal Form 103 na sasagutan ng punerarya.

b. Dalhin ang form, lahat ng medical records, at deceased retrieval form na may pirma ng health center sa barangay health center para sa interview.

c. Dalhin ang form kasama ang dahilan ng pagkamatay at iba pang dokumento sa City Health Office (CHO) para sa huling pagsusuri.

d. Kumuha ng Transfer Permit mula sa Sanitation Office at City Health Office (CHO) Room 5 (kung kinakailangan).

e. Pumunta sa Local Civil Registrar para sa rehistro ng death certificate.

B. KUNG NAMATAY SA OSPITAL:

a. Ang form ay na-type o nagawa na ng ospital (sanhi ng pagkamatay at kumpletong embalmer’s box).

b. Dalhin ang form sa City Health Office (CHO) Room 5 para sa huling pagsusuri.

c. Kumuha ng Transfer Permit mula sa Sanitation Office at City Health Office (CHO) Room 5 (kung kinakailangan).

d. Pumunta sa Local Civil Registrar para sa rehistro ng death certificate.
