MCR Office-Tampakan will have a scheduled transaction to PSA-Region XII to facilitate request of Security Paper or "PSA Copy" of CENOMAR, Birth, Marriage, and Death Certificates on Tuesday, 04 July 2023.

Requirements:
1. 1 Valid ID (Photocopy)
3. Authorization Letter (if not the parent or owner of document)
2. Processing Fee/s
PSA- ₱ 155.00
LGU processing fee - ₱ 116.60

*Requirements must be complied within this week or before 4 July 2023.
