โœ… Minutes of Meetings with Attendance Sheets (if available)
โœ… Justification of absences, if applicable
โœ… Activity Report with photo documentation
โœ… Copy of the CBYDP and the Resolution adopting it
โœ… Copy of the ABYIP/Annual Budget and the Resolution adopting it
โœ… Transmittal Letter to the MYDO
โœ… Other supporting documents, as applicable
โœ… Verification by the LYDO or confirmation from the concerned NYC Area Office
