The Oklahoma Rural Health Transformation Program has released a public grant application through a Notice of Funding Opportunity for the Rural Regional Reorientation (RRR) Program.

Applications are due by 11:59 p.m. CT on June 12, 2026.
RRR will provide grants of up to $4 million to eligible clinical and social service providers, either individually or in partnership with one or more entities.

This program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $223,476,948.62 with 100 percent funded by CMS/HHS. The Oklahoma Rural Health Transformation Program has released a public grant application through a Notice of Funding Opportunity for the Chronic Disease Management Program.

Applications are due by 11:59 p.m. CT on June 8, 2026.

The Chronic Disease Management Program is intended to fund providers, community-based organizations, and other organizations to implement and/or expand proven models for chronic disease management across rural regions of Oklahoma.
