NCHPH visits La Maestra Community Health Centers!

Last week, the National Center for Health in Public Housing had the opportunity to visit La Maestra Community Health Centers in San Diego, California. We are grateful for the warm welcome that our health center partners provided. During this site visit, NCHPH also had the opportunity to discuss what their health center needs are and what our continuing collaborations are aiming for to improve the health outcomes of their patient population.

In celebration of , NCHPH would like to commend WellSpace Health for the opening of their new Gregory Bunker Care Transitions Center of Excellence in South Sacramento in collaboration with Dignity Health, Kaiser Permanente, Sutter Health, and UC Davis Health. The recuperative care center will provide healthcare for those experiencing homelessness. The center will also offer those discharged from the hospital and who don't have stable housing, a space to fully recover and be better prepared to return to the community. The Special and Vulnerable Populations Diabetes Task Force, comprised of 13 National Cooperative Agreement (NCA) partners, have created "Diabetes in Special and Vulnerable Populations: A National Learning Series" to increase foundational knowledge of effective strategies to prevent, treat, and manage diabetes among special and vulnerable Health Center Program patients.
