NCHPH visits La Maestra Community Health Centers!
Last week, the National Center for Health in Public Housing had the opportunity to visit La Maestra Community Health Centers in San Diego, California. We are grateful for the warm welcome that our health center partners provided. During this site visit, NCHPH also had the opportunity to discuss what their health center needs are and what our continuing collaborations are aiming for to improve the health outcomes of their patient population.
NCHPH visits San Ysidro Health!
The National Center for Health in Public Housing would like to express gratitude and appreciation to San Ysidro Health for welcoming NCHPH's Program Director, Bob Burns and Training and Technical Assistance Manager, Fide Pineda, during a site visit last week in San Diego, CA. is looking forward to continuing with ongoing collaborations with San Ysidro Health to help improve the overall health outcomes for their patient population by addressing topics such as the Social Determinants of Health, public housing partnerships, improving access to care and more.

In celebration of , NCHPH would like to commend WellSpace Health for the opening of their new Gregory Bunker Care Transitions Center of Excellence in South Sacramento in collaboration with Dignity Health, Kaiser Permanente, Sutter Health, and UC Davis Health. In this upcoming Facebook Premiere, NCHPH hosted a dynamic introduction to the rich collection of resources on diabetes prevention and management for Health Center Programs. In this webinar, participants learned about existing resources pertaining to diabetes and received guidance and instruction on how to identify resources that fit individual Health Center needs.
As part of National Health Center Week, today is Health Care for the Homeless Day. Today, we will be featuring our webinar on housing and health care: Housing is Health Care: Prevalence and Considerations across the Housing Spectrum This webinar highlights the importance of housing as a social determinant of health by describing the health care needs and challenges of individuals that are homeless, live in supportive housing, or are public housing residents. In this webinar, panelists report and compare health care utilization and prevalence of diseases for these vulnerable populations and offer best practices on ways to improve access to care. This activity is part of an ongoing effort to provide training and technical assistance on issues related to housing and health.
