Muscle Stiffness in Dementia

Muscle stiffness can sometimes occur in people living with dementia, particularly as the condition progresses or when certain types of dementia affect movement and coordination.

A person may appear rigid, move more slowly, have difficulty standing up, walking, turning in bed, dressing, bathing, or completing other everyday activities. Their arms or legs may feel less flexible, and they may take shorter steps or become more cautious when moving.

Muscle stiffness is especially common in conditions that affect both thinking and movement, such as Lewy body dementia or dementia associated with Parkinson’s disease. However, stiffness can also result from reduced physical activity, arthritis, pain, medication side effects, dehydration, or another medical problem.

For caregivers, it is important not to assume that every movement change is simply part of dementia. New, sudden, painful, or rapidly worsening stiffness should be discussed with a healthcare professional, particularly if it is accompanied by weakness, fever, falls, confusion, or difficulty walking.

Gentle support can make everyday movement safer and more comfortable. Encourage regular movement when possible, allow extra time for transfers and walking, use supportive footwear, and consider physiotherapy or occupational therapy when appropriate. Avoid forcing stiff joints or pulling someone quickly into a new position.

Most importantly, remember that slower movement does not mean the person is unwilling to cooperate. Their body may simply need more time, support, and reassurance.
