It is with a heavy heart that we announce the closing of Identity Health Clinic. We recognize the anxiety this produces among our community, and we want to assure you that we will do everything in our power to provide a safe and stable transition for our patients.
In-person care will continue through the end of this week. May 1 will be our last day for scheduled telehealth appointments. May 15 will be the last day our team can directly help existing patients with transition needs such as medication refills, referrals to other providers, and medical records requests.
After May 15, Identity will no longer be able to provide ongoing patient support or services. However, patients will still be able to access their medical records after that date. We will share additional information soon about how to request records access.
Current patients have been notified via the patient portal. Patients are strongly encouraged to send a portal message for medication refills, referrals to other providers, or medical records requests. Please be as specific as possible in your message and allow up to 4 business days for our team to respond before calling.
Over the next few weeks, Identity will begin a fundraising effort to insure that patients and staff are supported during this transition. This fundraising will allow us to continue upholding our promise that no patient’s bill is turned over to collections.
