CCWC provides 18 unique services within four areas: Behavioral Health, Family and Children, Re-Entry Services, and Senior Services, providing high-quality, impactful service to some of the

most vulnerable persons in the surrounding communities and neighborhoods in Washtenaw County. CCWC's model of care focuses on addressing elements within the five social determinants of health (SDOH), thereby focusing on narrowing gaps in health disparities and inequities. Specific services include mental health therapy, substance use disorder treatment, domestic violence and anger management education, transition for recently incarcerated citizens, food pantry, as well as provision of childcare necessities and support of families to avoid homelessness, such as payment of utilities and rent assistance. Child programing includes foster care, adoption, pregnancy counseling, case management, and child victim advocacy. Senior Services include integrated resource advocacy and navigation, home safety, repair, and maintenance, generational support for grandparents raising their grandchildren, and Medicare and Medicaid enrollment assistance. The provision of this breadth of distinct services among the three program lines uniquely position CCWC to address complex needs of people within the community of all ages in a holistic way. Hello! We're Jake and Hannah. We've been happily married for 13 years. Jake owns a custom carpentry business, and Hannah is an operations coordinator. Spending quality time together, trips up north camping, and enjoying the outdoors with our hunting dogs are some of our favorite interests. We've been patiently waiting to add children to our home, being parents is something we've always felt in our hearts we were meant to do. Creating a home where a child can enjoy life, know they are unconditionally loved, and follow their passions to grow to their fullest potential, is our greatest hope.

We both love to read, play video games, and each have our own favorite anime's. We also enjoy balancing time spending it with family, friends, or quiet night ins.
