💚 We want to hear from YOU! 💚

We hope our Caregiver Chats have made an impact in your life—whether by providing helpful information, connecting you with others who understand, or simply giving you a place to ask questions and share your experiences.

Now, we’d love to hear how the Caregiver Chats have impacted you and how we can make them even better!

👉 Have you attended a Caregiver Chat? Tell us what you’ve enjoyed, what has been helpful, and what you’d like to see in the future.

👉 Never attended a Caregiver Chat? We want to hear from you too! What has kept you from joining us? Is there something we could change or improve that would make you more likely to participate?

Your feedback is important and will help us shape future Caregiver Chats to better meet the needs of caregivers in our community. 💚

🎁 As a special THANK YOU for completing the survey, you will be entered into a drawing for a $50 Kwik Trip gift card!

Thank you for taking a few minutes to share your thoughts and help us make Caregiver Chats even better! 🌟

Take the survey and let your voice be heard: https://docs.google.com/forms/d/e/1FAIpQLSccIgNDylZM9H8CzXr1oI--8nlXXUscYNkq5lFS6akV0ly0CA/viewform
