Connections to Care

Connections to Care A not-for-profit volunteer-based organization, we assist seniors with rides to/from medical appointments across NW suburbs.

To donate: www.gofundme.com/f/keeping-seniors-healthy Connections to Care is a 501(c)(3) not-for-profit volunteer-based organization, whose mission is to connect with older adults and help them care for their health and maintain independence by providing rides to and from medical appointments. Up to 30% of all patients m

iss their appointments due to lack of transportation, and this applies to our Senior citizen population as well. (Source: SCI Solutions, 2018)

All drivers with Connections to Care are volunteer drivers; they offer their time and resources to pick up a passenger, take them to their appointment, wait with them during their appointment, then drive them back to their residence. We cover a 160 square mile area within the Northwest Suburbs outside of Chicago, which includes 5 major medical centers. Additionally, if one of our passengers has an appt outside of our territory, such as a VA appointment, we will work with this passenger to find a driver to ensure they make it to their appointment. We operate on donations from the community, organizations and our passengers. We ask for a suggested donation of $15 for a round-trip ride within territory and $30 for a round-trip ride outside of territory. Our administration team consists of a volunteer board, a director, an operations manager and office volunteers, in addition to our volunteer drivers. We are a growing organization, and we all love what we do! We are so lucky to have the support and engagement of an amazing senior in our community - Ms. Pat, pictured below.

Ms. Pat has utilized our transportation services for almost 10 years - and through this time, we have become very close with her. We have transitioned through moving and had the joy of sharing excitement about her family. Her final move to southern IL this fall may have ended our ability to help, but she continues to be a pillar of support and care for us.

Two notable facts about Ms. Pat:
* She always called us her angels and my girls - a touching endearment from such a wonderful person
* She once told us "If I ever won the lottery, I would give all of my money to Connections to Care".

Our connection with Ms. Pat runs deep and we will always carry her with us.

We are so fortunate at Connections to Care to have the support and guidance of a wonderful board of volunteers, who continue to help us grow and support our seniors and volunteers!
