Every premium dollar is required by law to go mostly toward medical care, and the numbers show exactly where it lands.

Hospital and prescription drug costs make up 65 cents of every dollar, while doctor visits and other care make up about 19 cents, with the remainder covering administration, taxes, and a small profit margin.

Rates aren't set arbitrarily either. Actuaries project what a covered group will need the following year, and regulators review and approve those rates before they ever reach consumers.

Health care costs are growing faster than inflation, and that shows up directly in 2026 premiums.

Individual market shoppers are seeing some of the steepest increases, with silver plan premiums projected to rise more than 35 percent, while employer-sponsored plans are seeing more modest increases in the range of 6.7 to 8.9 percent.

Nearly 90 percent of that growth is tied to the rising underlying cost of care itself, meaning hospital, drug, and provider prices.

More than 25 million Texans have health coverage today, and most of them get it through their job.

Employer coverage remains the largest single source of protection in the state, followed by Medicaid, the individual market, and Medicare.

Each of these paths works a little differently, and understanding the mix is the first step toward understanding health policy in Texas.
