Marriage Certificates are only available at the Town or City Clerk's office where you originally obtained the Marriage License. You may also request a certificate from NYS DOH by filling out this form: https://cms8.revize.com/revize/miltonny/Document_center/Documents/Town%20Clerk/Documents/Application%20for%20Copy%20of%20Marriage%20Record.pdf. There may be a long delay with NYS.
