who will listen to your request for help and assist you with developing a plan to take care of your senior. We can give referrals for services and agencies for at home care.We can

you select to visit. We will educate you about the services, cost and the facility state rating. We can assist you with family care plan meetings and advocate for your love ones care at assisted living meetings and at nursing homes. Families need to start making a plan when their senior is in the hospital. Waiting for a DC date maybe to late. Hospitals discharge quickly and at any time of day or night, and often without warning. The hospital will send your love one to
the first nursing home that has an open bed. When Medicare days run out at the nursing home you will have to take your senior home, or pay out of pocket. What families do not know is
a large part of the deciding when Medicare days are up depends on what goals are set and
when your senior reaches them or no longer needs skilled nursing care. This is the reason
families need to attend care plan meetings with the goals they need the senior to reach to return to their home. If your senior is taken to hospital call us and let us help you make a plan.

Hi there just wanted to give up a few tips to follow,when you are in a panic because mom,dad,or your spouse became very ill suddenly and now that they are getting better ,the hospital is recommending long term
Care or nursing home. What do you do what is your response.First don't just agree,make them tell you why they feel it is necessary. Second and most important do Your own.assmement of pt problems, and care needs. Do you or family members have the time and space
To assist pt in your home or theirs.
There are a lot of devices that can help,but you need to assess the home first,and how to make it pt friendly.
