Position Title: Provider/Caregiver

Position Summary: Seniors Helping Seniors is currently seeking Adult Home Service Providers to work with other seniors in their homes. This means assistance with activities of daily living, light housekeeping, personal laundry, and companionship provided to an individual in his or her personal residence, which are intended to enable that individual to remain safely and comfortably in his or her own personal residence. Home Services does not include services that would be required to be performed by an individual licensed under the Nurse Practice Act. Home services are focused on providing assistance that is not medical in nature, but is based upon assisting the client in meeting the demands of living independently and maintaining a personal residence, such as companionship, cleaning, laundry, shopping, meal preparation, dressing and bathing.
Essential Functions:
1) Home Services Workers will provide services in accordance with the policies and procedures of Seniors Helping Seniors®, as well as the service arrangements spelled out in the Receiver Agreement (client contract).
a. Provide timely, written documentation on duties performed for client.
b. Maintain client confidentiality and privacy.
c. Participate in Provider educational processes.
d. Perform all other reasonably related duties as requested by the supervisor.
2) Duties of home services workers (Providers) may include the following:
a. Observation of Receiver/client functioning and reporting changes to his/her supervisor or employer;
b. Assistance with household chores, including cooking and meal preparation, light cleaning and laundry;
c. Assistance in completing activities such as shopping and appointments outside of the home;
d. Companionship and medication reminding;
e. Completion of appropriate records documenting service provision; and
f. Assistance with activities of daily living.
3) In addition home services workers shall not act in the following capacities:
a. Provide skilled medical personal care services as defined in Section 245.20 of the IDPH Administrative Code;
b. Become or act as a Power of Attorney;
c. Be involved in any solicitation of work or financial transactions of the client outside of contracted services. In such cases, the Provider shall follow Seniors Helping Seniors® policies with regard to any additional work/job requests from the client, and secure receipts for items purchased for the Receiver (client), ensuring both Receiver and Provider signatures document those expenditures in daily documentation;
d. Perform or provide medication setup for a client; and
e. Other actions specifically prohibited by Seniors Helping Seniors® policy or other State laws.
Qualifications:
• Does not have a disqualifying background check under the requirements of the Health Care Worker Background Check Act without a waiver,
• Can safely drive a motor vehicle, has a current driver’s license and proof of appropriate personal liability vehicle insurance, can provide visual proof of their Social Security card; or current passport, and has passed the initial health evaluation per IDPH and SHS-FV policy; Doctor’s written endorsement of their ability to work within scope of SHS-FV services to other Seniors and that they are free from communicable diseases, including TB, (this information will remain in Provider personnel file).
• Has visa or proof of citizenship in compliance with federal requirements for employment, (an I-9 form is signed by the Provider prior to their start date).
• Recommended to be over the age of 40 and consistently demonstrates patience, flexibility, good humor, and a compassionate, loving heart in order to help seniors who are in need of services from Seniors Helping Seniors® Fox Valley.
