On-site response by a licensed professional to behavioral health crises and other crisis behaviors that place the individual at increased risk for an institutional placement, loss of community placement, or otherwise threaten the health and safety of the member or the direct care staff
Supportive Living
Direct, in-home supports (also provided in community settings) in which members are accompanied by trained direct care staff who assist them on goals and objectives to increase independent living skills, ensure safety, and improve overall quality of life
Consultation Services
The identification of specific needs of the individual and barriers for safe, independent living accompanied by the development of plans by a qualified team of professionals to implement strategies to overcome barriers; plans may include Safety Plans, Positive Behavior Support Plans, Person-centered Service Plans, or Individual Program Plans - Consultation services also include the training of direct care staff, family members/guardians, or other parties who support the individual to equip the support team with skills needed to most effectively meet the needs of the member. Other services that may be accessed through the agency include supplemental supports, environmental modifications, and specialized medical supplies.
