🚨 Important CDCR Forms Families Should Know About

Many families don’t realize there are specific forms incarcerated people can complete that help ensure medical communication, emergency notification, and personal wishes are documented.

It’s also very important that your loved one request copies of any completed forms and send them to you so you have them for your records in case something ever happens.

Here are a few important ones to know:

CDCR 7385 – Authorization to release medical information to a family member or other person. Without this form, CDCR will usually not discuss medical conditions due to HIPAA.

CDCR 1019 / 1021 – Emergency contact forms used to document who should be notified in the event of serious illness, injury, or death.

CDCR 7421 – Advance Health Care Directive where a person can document their medical wishes if they become unable to make decisions.

CDCR 7465 – Do Not Resuscitate (DNR) request for individuals who choose not to receive CPR or resuscitation efforts.

CDCR 108 – Property inventory form documenting personal property.

CDCR 812 – Notice of Critical Illness or Injury used by CDCR when next of kin must be notified.

📌 Tip for families:

Encourage your loved one to review these forms, keep them updated, and send you copies so you have documentation if an emergency ever occurs.

The incarcerated person can request these forms from the medical office.

Yesterday, I visited Ironwood State Prison to congratulate the graduates of the Dog Training Academy.

These graduates taught their assigned dog meaningful skills preparing them for adoption. This program not only helps alleviate overcrowded kennels but also provides companionship for participants.

📢 Important Update: CDCR Tablet Distribution Schedule Has Changed

If you have a loved one incarcerated in a California state prison, please be aware that CDCR has updated the tablet distribution schedule, effective July 3, 2026.

As each institution transitions to the new Securus tablet system, the phone number that appears on your caller ID will also change. To help avoid accidentally declining a call from your loved one, be sure to save the new facility phone number before your institution goes live.

📱 A call from an unfamiliar number may simply be your loved one reaching out through the new system.

Please review the updated facility phone numbers and save the one for your loved one’s institution.

We know change can be confusing, but staying informed will help ensure you don’t miss an important call.

Statewide IFC Update

We wanted to provide a brief update regarding recent discussions about the CDCR budget.

CDCR has advised institutions that they may need to make operational adjustments through June 30, 2026. Facilities have been informed that staffing shortages and restrictions on overtime may impact certain institutional operations, including yard time, dayroom access, and programming. Each institution has been given discretion to make adjustments based on its operational needs.

At this time, visiting is not expected to be impacted by these budget-related adjustments.

As always, if we receive additional information or guidance from CDCR, we will share updates with local IFCs.

📢 SIFC Update Regarding California Budget Discussions Circulating
We are aware that there has been information circulating regarding potential budget reductions within California and CDCR.

At this time, please know that these potential budget cuts are not expected to impact visiting programs or family visitation services.

The Statewide Incarcerated Family Council (SIFC) is actively communicating with Headquarters regarding any developments and will continue to provide updates as information becomes available through official channels.

If you receive information from your institution indicating that programs, services, or activities are being reduced or eliminated due to budget concerns, please contact your assigned Statewide IFC Representative as soon as possible. This allows us to gather accurate information, seek clarification when necessary, and ensure concerns are addressed appropriately.
