Criteria for Participation

• Must be Medicaid eligible
• Must meet a level of care equivalent to an intermediate care facility (ICF-I)

Each Medicaid recipient has the freedom to choose their providers. The recipient’s choice of case management or waiver service(s) providers does not affect their eligibility for the waiver or case management services. Recipients of Medicaid Case Management Services may report any problems with their case management services or waiver services to the Bureau of Community Supports & Services Help Line Manager at

Here is How We Operate
The Client interviews and chooses the person with whom he/she feels most comfortable. The client is in COMPLETE control of scheduling and has the CHOICE to refer a personal care attendant in their COMMUNITY to provide CARE, for those who meet employment qualifications.

General information

Programs and Services Offered Children’s Choice Waiver (CC) This program provides services to children with developmental disabilities up to 18 years of age. Services provided include environmental adaptations, family training, diapers incontinence briefs, and family support. Family support services can be provided in the home, after school programs, summer camps, or other specified locations. CC provides assistance with daily care, self-care, meal preparation and participating in community services. Early Periodic Screening Diagnostic Treatment (EPSDT) This program provides services to children up to the age of 21 years. Services are provided to individuals in their home. EPSDT-PCS provides assistance with eating, bathing, dressing, grooming, laundry, meal preparation, etc. Elderly Disabled Adult Waiver/Personal Assistance Services (PAS) This program provides services to individuals with disabilities who are age 21 or older. Services provided include Case Management, Personal Care Services, Companion Care, Personal Supervision(Day & Night), Supported Independent Living Skills, and Home Modifications. EDA provides assistance with leisure, Health and Safety, Self-care, Home living, and Communication. Long-Term Personal Care Services (LT-PCS) This program provides services to Medicaid participants who are either 65 years of age or older with disabilities. Services provided include ADLs (eating, bathing, dressing, grooming, ambulation, transferring, and toileting) & IADLs (laundry, light housekeeping, grocery shopping, meal preparation, accompanying to medical visits, etc.) New Opportunity Waiver (NOW) This program provides services to children at least 3 years of age with MR or developmental disabilities. Services provided include assistance/prompting with personal hygiene, dressing, bathing, grooming, eating, assistance in maintaining a safe/healthy home, and personal support in community health and leisure activities.
