We are looking for an organized, passionate, and energetic new team member:
Aging Life Care Manager
Smith Integrated Care Services strives to assist and
accommodate seniors who are experiencing diseases of
aging and dementia symptoms, and their families, in the
visualization and implementation of a future that is dignified,
joyful, and empowered.

Smith Integrated Care Services strives to maintain a culture of
accountability, respect, gratitude, and compassion.
Effective performance of this role includes the following:
● Participate in weekly Case Review Meetings to develop background knowledge regarding individual cases and plan for the upcoming week of care management.
● Review electronic medical records to ensure:
○ Appointments are attended.
○ Physician communications and test results are responded to timely.
● Complete quarterly assessments for long-term clients.
● Schedule & facilitate quarterly team meetings when care plans are reviewed.
● Document billable and nonbillable time in appropriate electronic systems.
● Complete comprehensive assessments for new cases.
● Maintain a full caseload of clients for which you provide 125 hours/month of concierge care management.
● Represent yourself and the company in a professional and respectful manner at all times.
● Provide coverage for team members as needed.
● Complete certifications and training as agreed upon to increase knowledge and abilities to support clients.
● Prepare intake packets.
● Other tasks as required.
Coming in to this role, you will need to have the following:
● Master’s degree
● Driver’s license with clean driving record.
● Reliable transportation; comfortable driving in urban and suburban areas around Boston.
● Passion for working with vulnerable populations and advocating for needs to be met.
It would be helpful if you had:
● Professional licensure (nurse, social worker, PT, OT, SLP, BCBA)
● Experience working with dementia patients
Compensation: $35-$45/hour depending on experience and qualifications, fully covered health insurance, vision and dental, HSA match, 401k match, group term life insurance, and professional development and certification opportunities throughout the year. This is a full time, salaried position.

The Benefits of Laughter

Researchers believe that laughter can provide physical, mental, and social Benefits.
Laughing helps the immune system by releasing neuropeptides that fight stress and serious illness.
It has the ability to reduce emotional and mental anguish by dropping levels of stress hormones, cortisol and adrenaline.
A social benefit that comes from laughter is the improvement of relationships, particularly caregiving relationships. Laughter releases tension and creates positive memories, which can be particularly strengthening in a caregiving relationship with someone who has memory loss. People who can't remember facts are often still able to remember the emotions they feel about a situation.
Laughter also helps caregivers' health and improves their feeling about caregiving. It preserves a relationship that is being deteriorated daily by the caregiving need. Being able to laugh with someone you are caring for gives the relationship another dimension of joy while also releasing chemicals in the brain that fight stress.
