Have you filled out the COVID-19 vaccine attestation form yet? If you haven’t, fill it out as soon as possible to make sure you are able to attend on-campus activities, on-campus classes, on-campus research, and live on campus. Fill it out once you are fully vaccinated or if you have an exemption. If you have any questions about the form, please contact the Wellness Center ([email protected]). The attestation form is the official form used by the BSU Wellness Center to keep track of who received the COVID-19 vaccine and those that are exempt from receiving it. Do I need to fill it out? If you are a student planning to come onto campus for any reason, such as living on campus, conducting research, taking in-person classes, and participating in ANY on-campus activities, you must fill out this form to show you are fully vaccinated or are exempt from receiving the vaccine. From today, February 22nd until March 26th, you can fill out the candidate application form to join Student Government as a Senator or even an Executive Board member. It is time to Listen. Advocate. And Change.
