Autumn Grove (Transitions Healthcare) 2021 Awardees (L to R): Mary Ann Hughes, Volunteer; Lisa Fonner, Activities Director; Lorie Frederick, RN.

Mary Ann became a patient advocate for staying in touch with the outside world on the onset of the 2020 COVID Pandemic. She gathered community members from the Harrisville area (who came in forces) to decorate the outside windows for the residents in the early spring of 2020. Mary Ann also collected numerous donations for the residents for Christmas.

Lisa demonstrates selflessness and patience very day with the residents. She loves to listen to their stores reminiscing about life experiences that have made them who they are today. Her heartfelt words of encouragement, thoughtful gestures of affection and compassionate acts of generosity are with Lisa incorporates into her daily routine.

Lorie has shown great leadership at Autumn Grove, taking in new hires under her wing, keeping the center running like a well-oiled machine on her pm shift, and is a strong team member who can be counted on until the job is completed!

Quality Life Services Awardees (Photo 1): Pictured left to right: Sue Tack and Kaitlin Smith. Sue is a volunteer who has been with "Don't Stop Dreamin" since the beginning (approx. 10 years). She is a retired Activities Director and always had a heart for the residents. She works tirelessly to make sure each dream is fulfilled.

Kaitlin Smith is a Clinical Reimbursement Specialist and brings a cheerful and helpful attitude to work with her every day. Kaitlyn always finds a way to help and is an expert at her job.

Karlie is a nurse who takes her role in her residents' lives very seriously. She is always looking for ways to provide individual and compassionate care. Dawn takes pride in keeping the facility clean and to the highest standards (especially through this time of COVID) and goes above and beyond to ensure the residents are kept safe, with a clean home; Diane Fair – Environmental Services Director / Supervisor.

Emma Starr was one of three Awardees for Health Care Student of the Year. Emma is a student enrolled at the Butler County Vo-Tech School as a Health Assistant Student. Emma plans to attend nursing school at the Butler County Community College. Emma is currently working on both 2nd and 3rd year certifications and serves as secretary for HOSA, a school club.
