I saved a patient’s life at 2 a.m., operating all night while she was comatose with a breathing tube down her throat. Two weeks later, she was out of the hospital walking and talking—but she was upset about her haircut.

The truth is, I don’t blame her. My mind does exactly the same thing. I can spend all night treating a massive brain hemorrhage and then get genuinely pi**ed off because my Amazon package is running late.

This is just what minds do. No matter how much gratitude or perspective I have, it feels like the human mind doesn’t have an “arrival setting”. It only has a “what’s next” setting.

Ever felt like you’re just pretending to be normal?

During my neurosurgery residency, I once walked into a holiday party after weeks of extreme sleep deprivation, battling shingles and sties in both eyes. While everyone around me chatted about vacations and TV shows, I was just trying to maintain eye contact and keep my nervous system from redlining.

It felt like I was the only one “performing normal” in a room full of humans. But looking back, I realize we’re all often just trying to keep it together.

We know the truth, yet we still chase the upgrade.

I spend my days seeing exactly how fragile life is. I’ve stood at the bedside when things get real, and I can tell you nobody is reaching for their car keys in those moments.

But knowing the truth doesn’t always change the desire. Even though I know the dopamine hit from a new car only lasts a week, my mind still tries to convince me that the next one will finally be the thing that fulfills me.

It’s a constant battle between what we know and what we want.

I think perspective lasts about 20 minutes.

I just had to tell a 50-year-old man that he has brain cancer. I watched as his mind completely reorganized and reprioritized his life. In that moment, I felt that same shift regarding what really matters and what doesn’t.

Then, 20 minutes later, I got in my car to drive home to my wonderful family. I immediately lost that perspective when someone cut me off getting onto the freeway.

Yeah, perspective lasts about 20 minutes.

it’s easy to feel like you have it all figured out when life is heavy, but even easier to let it slip away over something as small as traffic. we often don’t realize how quickly our priorities can shift back to the trivial, even after a moment of profound clarity.
