Our dental team sent forms home in your child's Back-to-School packet. If you would like for your child to participate, please complete the form and return it to school.
✨There is NO COST to parents for these services!
Our schools dental peogram is designed to help your child's oral health and provide convenient preventative dental services right at school.
🪥IMPORTANT: This program DOES NOT take the place of your child's regular dentist. We encourage all children to continue seeing their family dentist for routine dental care and treatment they may need.
📝 Check your child's Back-to-school paperwork for the form pictured below, complete it, and send it back to school!
