The Connecticut State Firefighters Association (CSFA) Membership in CSFA is open to any active Connecticut Firefighter over the age of sixteen. A non-profit organization since 1885

Founded in 1885, the CSFA represents the interests of more than 26,000 Firefighters across Connecticut; 22,625 Volunteer Firefighters and 4400 Career Firefighters.

Updated CSFA Forms Now Available Online

The Connecticut State Firefighters Association (CSFA) announces that all Association forms have been updated and are now available on the official CSFA website at www.csfa.org.

All documents are provided as fillable and downloadable PDF forms to ensure ease of completion and submission.

The following forms are currently available:
• Injury Claim Application
• Death Claim Application
• Total Permanent Disability (TPD) Application
• Exposure Report Form
• Hall of Fame Nomination Form
• Cancer Relief Fund Application

The Line of Duty Death Application is located on the Connecticut Fallen Firefighters Memorial website at www.ctffm.net.

All completed forms may be submitted electronically via email or mailed to the new CSFA mailing address at the Connecticut Fire Academy:
