📢 We're hiring for a Contractual Social Worker support both our Adult Evaluation and Review Services (AERS) Program and Medical Adult Day Center (MADC)!

This unique position provides an opportunity to make a meaningful impact in the lives of older adults and adults with disabilities by:

⏩Conducting assessments and care planning
⏩Coordinating community services and supports
⏩Assisting individuals and families in navigating long-term care resources
⏩Working as part of an interdisciplinary team to promote independence and quality of life

Preferred Qualifications:

⏩Experience working with older adults and adults with disabilities
⏩Strong assessment, case management, and care coordination skills
⏩Knowledge of long-term services and supports, Medicaid, and community resources

Candidates must be licensed as a Graduate Social Worker, Master Social Worker, or Certified Social Worker by the Maryland State Board of Social Work Examiners prior to appointment to a position in State service.
