Last week I sat at a kitchen table with a mom in her mid-30s, her caregiver close beside her, while her two little ones played in the next room. We were there for one reason: to get her advance directive and healthcare power of attorney signed and notarized, so the people she trusts can carry out her wishes if she's ever unable to speak for herself.
