1️⃣ Head to our website & fill out a brief questionnaire exploring eligibility --> https://www.soldiersandfamiliesembraced.org/our-support

2️⃣ Fill out our brief Intake Documents --> Within 24 business hours you will receive an email from SignNow to complete intake documents.

3️⃣ Complete Intake Assessment --> Once intake documents are received, you will receive an email to schedule a 15-minute intake appointment via Zoom. Once this is completed, we will assess what counselor to assign you to.

Join us on July 17, 2026 at Swan Lake Golf Course in Clarksville, TN for our annual fundraiser! 100% of profits goes back towards providing mental health care options for our Veterans, Active Duty, First Responders and their families.
