Michelle just celebrated 1 year on our team, and in that time, she’s been absolutely instrumental in helping our agency grow and thrive. From searching for housing and securing heating and transportation resources, to applying for grants for assistive technology, supporting participants, and even lending a hand with their pets—Michelle has gone above and beyond in every way.

We are so grateful for her dedication and all she brings to our team and the people we serve. 💛

Department of Health has released the draft for amendments to the Nursing Home Transition & Diversion Waiver Program. Part of this includes caps on enrollment. They are allowing public comments to be submitted until 7/18/25. If you are interested in learning more and advocating please DM me!

Overview:
We are seeking a dedicated Service Coordinator to join our team. As a Service Coordinator, you will play a crucial role in managing and developing programs aimed at enhancing the well-being of individuals who have sustained brain injuries and the aging population. This is a per diem/part-time position with the option to grow into full-time. Therefor income varies based on caseload size.

Responsibilities:
- Oversee the development and implementation of programs tailored to meet the needs of individuals served.
- Manage relationships with clients, families, and provider agencies to ensure effective program delivery.
- Work closely with a team to supervise program activities and ensure compliance with DOH standards
- Engage in tasks such as writing case notes, making referrals and linkages, developing plans and in person visits.

Qualifications:

Must have a Bachelor’s degree and one (1) year of experience providing case management/service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors;

OR Individual with an Associate’s degree and two (2) years of experience providing case management/service coordination, information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors.
