"Despite all of the excitement about value-driven care, there is a remarkable lack of clarity about what “value” means in this context. Moreover, though providing better outcomes at lower costs may be a noble goal for a health care organization, a system organized around competition to achieve this objective is not functional in a market-based health care economy." "It is also completely unclear how competition based on value, defined as the “health outcome per dollar expended,” will succeed in lowering costs. Organizations can certainly succeed in lowering costs internally using Lean production methods, but in the marketplace, health organizations compete on their ability to make money."

A really good article written by a medical student Nathan Favini. It has been proven that The most expensive health care offered doesn't equate to good outcomes. Read the entire article at: KevinMD.com

I am my best evidence about why the presence of a Care Navigator at every Doctor visit can make a difference in understanding and compliance.
I had major back surgery a week and a half ago. When I went to my follow up appointment yesterday. I was prepared with the information he needed to know and the questions I needed answered.
The Dr came in the room and began talking. He answered all my questions, told me when to come back and it was over. As my husband and I walked out of the office I looked at him and said "Do you remember anything he told me to do?" My husband looked at me and said "you do this for a living you didn't take notes and now you can't remember?"
I was too busy being the patient and didn't catch and retain about 50% of what was said.
I should have had a Care Navigator. How much do you remember from your Drs appointments?

People in the United States think that we have the greatest health care in the world. Having all of your medical directives discussed, signed, and copies sent to a loved one will insure that your health care wishes are followed. What should you have?
Medical Power of Attorney
A document, signed by a competent adult, i.e., "principal," designating a person that the principal trusts to make health care decisions on the principal's behalf should the principal be unable to make such decisions. The individual chosen to act on the principal's behalf is referred to as an "agent."
Be sure that your "agent" knows or has in writing how your care is to be handled if you are unable to speak for yourself.
