REGISTRATION SCHEDULE:
Address: 2525 Illinois Avenue Columbus, Indiana 47201
Days: Mon-Thurs Oct 12-15 Oct 19-22 Oct 26-29
Time: 9:00 AM – 3:00 PM

REQUIRED ITEMS FOR REGISTRATION:
• ID for all adults in the home
• Birth Certificate for all children residing in the home
• Children newborn through age 15 only
• Newborns must be born by December 25, 2026
• Proof of guardianship for those children who are not listed as parent on Birth Certificate
• Proof of address (utility in your name)
• Proof of INCOME (employment, unemployment, Gov’t Assistance, Food Stamps, SS, SSI, etc)
• Proof of EXPENSES (rent, utilities, phones, child support, auto, all monthly expenses)
• Three WISH LIST items per child ($40 or less each)
• Clothing and shoe sizes for all children
• No bicycles or electronics, please

ADDITIONAL INFORMATION:
• On-line Registration is not available. Sign-up is in person only
• Please do not bring children over age 4 to registration
• Applications are not accepted after October 29, unless a letter from Case Worker, Physician, or Pastor, which will be verified
• You may not apply for Angel Tree gifts for children whom you do not have legal guardianship. No grandchildren, nieces, nephews, friend’s children, etc

Monday, Oct 19- Thursday, Oct 22
Monday, Oct 26- Thursday, Oct 29

Applications will not be taken after October 29 unless there is a verifiable emergency.
