Services Provided:
Supported Community Living (SCL) provides one to twenty-four hours of support per day based on the individual’s needs. Assistance may include, but is not limited to: personal and home skills, community skills, personal needs, transportation, and treatment services. Consumer-Directed Attendant Care (CDAC) provides assistance to individuals with self-care tasks, which they would
typically do independently if they were otherwise able. Home-based Habilitation Services are individually tailored supports that assist with the acquisition, retention, or improvement in skills related to living in the community. These supports may include adaptive skill development, assistance with activities of daily living, community inclusion, transportation, adult educational supports, social supports and leisure skill development. Day Habilitation Services are services that assist or support the consumer in developing or maintaining life skills and community integration. Prevocational Habilitation services include teaching concepts such as compliance, attendance, task completion, problem solving, and safety. Services are not oriented to a specific job task, but instead are aimed at a generalized result. Supported Employment Habilitation services are intensive, ongoing supports that enable individuals to perform in a regular work setting. Basic individual respite means respite provided on a staff to consumer ratio of one to one or higher for individuals without specialized medical needs that would require care by a licensed nurse. Staff can do minor repairs around the house, lawn mowing, snow removal, deep cleaning or other projects, depending on the individual’s need. Prevocational Services are aimed at preparing an individual for employment, but are not job-task oriented. These services include teaching the consumer concepts such as following directions, attending to tasks, task completion, problem solving, and safety and mobility training. Supported Employment services are individualized services associated with obtaining and maintaining competitive paid employment in the least restrictive environment possible, provided to individuals who need intense and ongoing support to perform in a work setting.
