The Nutrition Program provides an average of 50,000 meals to older adults each year. Older persons interested in Meals on Wheels are assessed and must meet eligibility requirements. Per the Federal Older Americans Act, meals are served on a donation basis, which allows individuals to pay what they feel they can afford, thus targeting services to the most economically disadvantaged. Suggested donat
ion per meal is $5.00. Rural buses and meal site vans provide an average of 5,000 one way trips per year throughout the county for various trip purposes, including medical appointments, nutrition, and personal business/shopping. The Volunteer Medical Escort Program serves those frail elderly unable to ride the rural bus. Volunteer drivers provide transportation to medical appointments using their own vehicles and donating all of their time. The Elder Benefit Specialist Program assists older persons regarding public benefits. On average, over 700 seniors per year are assisted with homestead tax credit, energy assistance applications, Medicaid, Medicare, Social Security & SSI, SeniorCare (Wisconsin Prescription Drug Program), and other topics and issues. Information and Assistance Program links older persons and their families to available opportunities and services. Up-to-date information is provided to those requesting assistance; an average of over 14,000 contacts per year are made, providing information. Follow up and referrals are made as needed. The Alzheimer's Family Caregiver Support Program provides coordination, assessments, and referrals for caregivers of family members diagnosed with Alzheimer's disease or other related disorders. Limited state dollars are available to assist families with respite care, supportive home care, and other goods and services. The National Family Caregiver Support Program offers information and assistance to family caregivers of persons 60 and older. Limited funding is available for respite and supplemental services for those meeting eligibility requirements. A six week training “Powerful Tools for Caregivers” is also held throughout the year. Caregiver Connection, a support group for caregivers, is held monthly. CHAIN Reaction Day is a county-wide volunteer project held in the Spring and Fall to help elderly with outdoor chore services including raking and window washing. This cannot be accomplished without our many dedicated volunteers throughout the county. Other programs provided include:
Stepping On Program: A seven-week workshop with guest speakers to reduce the risk of falls and increase quality of life. Living Well with Chronic Conditions: A six-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma, and heart disease. Powerful Tools for Caregivers: A six-week workshop to help identify and manage the physical, emotional, and financial challenges that family caregiving can present and help caregivers connect with others facing similar feelings and problems. Mind Over Matter: A three-session workshop designed to help women take control of bladder and bowel incontinence. Age Well Without Falls: A three-week falls prevention class. Stand Up & Move More: A five-week workshop to help older adults reduce sedentary behavior.
