Care Coordination Network - SWCAP consists of Community Health Workers who meet with individuals one-on-one to assess their specific needs and connect them to resources such as medical and dental providers, behavioral health providers and other social service programs such as food pantries, housing and transportation to address SDOH barriers. Not only do Community Health Workers connect individuals to services, they follow up to make sure needs are met on a continuous basis.

Weather Update – Travel Impacts Possible Today

This Morning:

Rain showers will move through the area early, but temperatures will fall quickly behind the rain.

This Afternoon & Evening (2 PM – 8 PM):

Scattered snow showers are expected to work through the area. At this time, I’m favoring light snowfall totals (generally under 0.5"), but road impacts remain a concern.

Why impacts matter:

With temperatures cold enough, snow may accumulate on pavement, especially on bridges, overpasses, and untreated roads.

Overnight:

A refreeze risk will be in play as temperatures remain cold. Winds will stay gusty, but I’m not confident they’ll be strong enough to fully dry road surfaces, so slick spots may linger.

Bottom line:

Even light snow can cause hazardous travel conditions. Slow down, allow extra time, and stay alert—especially during the evening commute and overnight.
